Scholarship Application Requirements: Filled out application completely which includes this online application and the following items: 1) Tax documentation 2) Athlete letter: a letter written by the athlete on why they would like to be part of SMBA 3) Parent letter: a letter written by parent/guardian stating why they need financial help with their program fees, and why they would like their family to be part of SMBA
